The party of science?

I’ve posted links on Facebook to a couple of articles pointing out that there’s no evidence that the lockdowns (or Coronapocalypse, if you like) have done anything to “flatten the curve”. It’s interesting to watch the reactions to these posts. Almost all have proclaimed “Of course it worked. See how the curve has flattened?”

To which I reply, “post hoc, ergo propter hoc”. Just because B follows A doesn’t mean A caused B to happen.

So for example, the cities that have high numbers of coronavirus cases, and where lockdowns have been more pronounced, also have large Neopagan communities. You can be sure the Wiccans have been gathering in living rooms (10 at a time or fewer) to cast spells to drive away the virus. And it must have worked, because see how the curve has flattened?

How would we know whether lockdowns actually save lives? Well one approach is to compare areas that did lock down against areas that didn’t. For example, what this piece in the Wall Street Journal does.

It seems that whether or not a region locked down accounts for some 5% of the variation in the number of deaths per million people. Now a lot more analysis is in order, and I have to be a little suspicious of someone who seems unable to keep the correlation coefficient and the coefficient of determination straight. But his work points in an interesting direction, and is at least an attempt to address the question in an objective manner.

OK, I’ll violate some copyright. (I thought this might be among the coronavirus pieces outside the paywall.)

Do quick shutdowns work to fight the spread of Covid-19? Joe Malchow, Yinon Weiss and I wanted to find out. We set out to quantify how many deaths were caused by delayed shutdown orders on a state-by-state basis.

To normalize for an unambiguous comparison of deaths between states at the midpoint of an epidemic, we counted deaths per million population for a fixed 21-day period, measured from when the death rate first hit 1 per million—e.g.,‒three deaths in Iowa or 19 in New York state. A state’s “days to shutdown” was the time after a state crossed the 1 per million threshold until it ordered businesses shut down.

We ran a simple one-variable correlation of deaths per million and days to shutdown, which ranged from minus-10 days (some states shut down before any sign of Covid-19) to 35 days for South Dakota, one of seven states with limited or no shutdown. The correlation coefficient was 5.5%—so low that the engineers I used to employ would have summarized it as “no correlation” and moved on to find the real cause of the problem. (The trendline sloped downward—states that delayed more tended to have lower death rates—but that’s also a meaningless result due to the low correlation coefficient.)

No conclusions can be drawn about the states that sheltered quickly, because their death rates ran the full gamut, from 20 per million in Oregon to 360 in New York. This wide variation means that other variables—like population density or subway use—were more important. Our correlation coefficient for per-capita death rates vs. the population density was 44%. That suggests New York City might have benefited from its shutdown—but blindly copying New York’s policies in places with low Covid-19 death rates, such as my native Wisconsin, doesn’t make sense.

Sweden is fighting coronavirus with common-sense guidelines that are much less economically destructive than the lockdowns in most U.S. states. Since people over 65 account for about 80% of Covid-19 deaths, Sweden asked only seniors to shelter in place rather than shutting down the rest of the country; and since Sweden had no pediatric deaths, it didn’t shut down elementary and middle schools. Sweden’s containment measures are less onerous than America’s, so it can keep them in place longer to prevent Covid-19 from recurring. Sweden did not shut down stores, restaurants and most businesses, but did shut down the Volvo automotive plant, which has since reopened, while the Tesla plant in Fremont, Calif., was shuttered by police and remains closed.

How did the Swedes do? They suffered 80 deaths per million 21 days after crossing the 1 per million threshold level. With 10 million people, Sweden’s death rate‒without a shutdown and massive unemployment‒is lower than that of the seven hardest-hit U.S. states—Massachusetts, Rhode Island, Louisiana, Connecticut, Michigan, New Jersey and New York—all of which, except Louisiana, shut down in three days or less. Despite stories about high death rates, Sweden’s is in the middle of the pack in Europe, comparable to France; better than Italy, Spain and the U.K.; and worse than Finland, Denmark and Norway. Older people in care homes accounted for half of Sweden’s deaths.

We should cheer for Sweden to succeed, not ghoulishly bash them. They may prove that many aspects of the U.S. shutdown were mistakes—ineffective but economically devastating—and point the way to correcting them.

Defenders of coronavirus lockdown mandates keep talking about science. “We are going to do the right thing, not judge by politics, not judge by protests, but by science,” California’s Gov. Gavin Newsom said this week. Michigan Gov. Gretchen Whitmer defended an order that, among other things, banned the sale of paint and vegetable seeds but not liquor or lottery tickets. “Each action has been informed by the best science and epidemiology counsel there is,” she wrote in an op-ed.

But scientists are almost never unanimous, and many appeals to “science” are transparently political or ideological. Consider the story of John Ioannidis, a professor at Stanford’s School of Medicine. His expertise is wide-ranging—he juggles appointments in statistics, biomedical data, prevention research and health research and policy. Google Scholar ranks him among the world’s 100 most-cited scientists. He has published more than 1,000 papers, many of them meta-analyses—reviews of other studies. Yet he’s now found himself pilloried because he dissents from the theories behind the lockdowns—because he’s looked at the data and found good news.

In a March article for Stat News, Dr. Ioannidis argued that Covid-19 is far less deadly than modelers were assuming. He considered the experience of the Diamond Princess cruise ship, which was quarantined Feb. 4 in Japan. Nine of 700 infected passengers and crew died. Based on the demographics of the ship’s population, Dr. Ioannidis estimated that the U.S. fatality rate could be as low as 0.025% to 0.625% and put the upper bound at 0.05% to 1%—comparable to that of seasonal flu.

“If that is the true rate,” he wrote, “locking down the world with potentially tremendous social and financial consequences may be totally irrational. It’s like an elephant being attacked by a house cat. Frustrated and trying to avoid the cat, the elephant accidentally jumps off a cliff and dies.”

He cautions against drawing broad conclusions about the efficacy of lockdowns based on national infection and fatality rates. “It’s not that we have randomized 10 countries to go into lockdown and another 10 countries to remain relatively open and see what happens, and do that randomly. Different prime ministers, different presidents, different task forces make decisions, they implement them in different sequences, at different times, in different phases of the epidemic. And then people start looking at this data and they say, ‘Oh look at that, this place did very well. Why? Oh, because of this measure.’ This is completely, completely opinion-based.”

People are making “big statements about ‘lockdowns save the world.’ I think that they’re immature. They’re tremendously immature. They may have worked in some cases, they may have had no effect in others, and they may have been damaging still in others.”

I was going to do this last night, but Monday’s chemo day, and Tuesday’s when I really want to get to bed early. (Today too, but not as much.)

I really would like to see some statistics and anecdotes about how Sweden is faring, because they have essentially done nothing but advise people to take common sense precautions and isolate the vulnerable.
